Documentation

Initiating the cremation process, including compliance with applicable laws and regulations, requires completing a number of documents, some of which can be done in advance. The forms are described below and available for download, or if you prefer, Elements Cremation will provide the forms to you in hard copy. We also provide full support and assistance to ensure all documentation is completed and filed in a timely manner and with as little inconvenience to you as possible.

Unless completed in advance, all forms must be completed by the closest living next-of-kin or other individual having legal authority. Whether completed in advance or at time of need, all forms should be returned to Elements Cremation in person, fax, or email. 

Elements Cremation uses the information to file for the legal permits required to begin the cremation process, including filing for the certified copies of the death certificate. Once all necessary documents are filed and permits received the cremation may take place.

Required Documentation

Statistical Information Form

The Statistical Information Form contains all the information necessary to complete the death certificate. This information is kept confidential and provided only to the various state and county offices required to certify the certificate.

Cremation Authorization

The Cremation Authorization allows Elements Cremation to obtain the necessary cremation permits from the county and directs the crematory to initiate the cremation process.

Authorization to Remove

The Authorization to Remove allows Elements Cremation to pick up the decedent from the place of passing or subsequent location. For example, if an individual dies at home this authorization allows Elements Cremation to pick up from the decedent’s home; if the individual were to pass at a hospital this authorization instructs the hospital to release the decedent into Elements Cremation’s care.

Obituary Form

Use the Obituary Form to organize and record information that can be used in public media to provide notice of passing and to memorialize the life of the loved one.
